TSOS Outreach Case Manager

Purpose:

Act as a responsible and reliable part of the TSOS to provide oversight, assistance, and reassurance to the participants of HRM Temporary Safe Outdoor Space (TSOS), maintaining a safe atmosphere within the TSOS structure while speaking truthfully with love.

Provides tailored case management to clients who access the temporary safe outdoor space participants.

  • BA in Social Services or related field preferred; a combination of college education and case management experience
  • Experience working with low-income, diverse populations preferred
  • Operate computer to execute diverse functions with speed and accuracy
  • Proficiency in google docs and word
  • Excellent verbal, written, and electronic communication skills
  • Ability to work as a team member, inspiring trust and speaking the truth
  • Able to respond calmly and effectively when under stress and interacting with people who have high barriers, including but not limited to people with mental illness, who may be on drugs, or be prone to violence.
  • Job-specific qualifications: Valid driver’s license, good driving record, proof of insurance

Direct Work

  • At the initial case management session, assess the client’s strengths, goals, and barriers. to housing and self-sufficiency
  • Document client information.
  • Formulate a written tailored plan based on client goals that identify specific
    action steps the client needs to take.
  • Meet with clients on their caseload a minimum of once each week to monitor
    Progress in achieving goals, and revising action plans when necessary.
  • Establish a hard copy file for each client that includes verification of Homelessness, verification of income and income source, and documentation of case plans and action steps. These files remain the agency’s property and must be kept on-site in a secure area.
  • Keep informed of current community resources that can help clients.
  • Attend all training sessions recommended by Hope Rescue Mission including but not limited to Cultural Competency, Motivational Interviewing, and Trauma Informed Care.
  • Perform other duties as requested by the Director of Outreach.
  • Compassionate and able to relate to different clients with various needs
  • Motivational to encourage clients to follow their care plans
  • Strong verbal and written communication skills to explain to clients, family members and friends, and professionals the case and care program and maintain good case records
  • Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from health care and social workers
  • Flexibility to change care plans if they are not getting the best results
  • Organization to manage several different cases at once
  • Computer literacy to maintain and manage case records
