PATH: Projects for Assistance in Transition from Homelessness
Projects for Assistance in Transition from Homelessness (PATH) is a federal program that provides outreach and engagement to people experiencing homelessness or at imminent risk of doing so. The services below are offered to people who have enrolled voluntarily in the program and have been diagnosed with severe mental illness.
Case management, medication management, and prescription renewal.
Referral to community behavioral health services
Referral to primary health and dental services, job training, education and housing services.
Screening and diagnostic services
Habilitation and rehabilitation
Enrollment in mental health services
Assistance with food stamps, social security, and housing applications
